SRP 4YR old- Kindergarten Registration form

It's time for the Summer Reading Program again! Join us on Monday's if your child(ren) are going into 4 year old preschool through Kindergarten. We'll start at 10 am and end at 11 am from July 10th through July 31st. Please make sure your child is register with us so that we have all of the contact information. Please call the library with any questions you may have. Parents do not need to stay with the child through the program, but we do ask that you pick them up within 15 minutes of the program ending.

You can find the registration form to the right of this box, please fill it out and return it to the library. You can also email it to director@atkins.lib.ia.us

Please have the form back to us by July 1st, thank you!
